We have an excellent, full time opportunity available for a Family Nurse Practitioner or Adult/Geriatric Nurse Practitioner to develop and implement an Annual Wellness Visit Program in Corvallis and Albany, Oregon.  The focus is on providing the absolute highest level of quality patient care and the staff is very friendly and team oriented to work with.  There is plenty of room for professional growth and advancement and a great mentoring environment is provided.

• Provide annual wellness visits and comprehensive patient assessment
• Design and implement individualized plans of care
• Ensure that patients and caregivers are educated on managing their conditions

QUALIFICATIONS

• At least 1 year of NP experience is required, geriatric experience is preferred
• Experience with CMS compliant AWV’s highly desirable
• Unrestricted Oregon NP  license (or eligible) is required
• Board Certification – AANC or AANP
• Valid Oregon driver’s license and reliable transportation
