Ready for a change?  We have an excellent, full time opportunity available for a nurse practitioner or physician assistant to join our team in Williamsport, Pennsylvania.  You will be partnering with other LTC teams in the region in order to improve the transition of care between the skilled nursing facility and the hospital. You will be working in collaboration with APPs as part of an established team that is dedicated to reducing inpatient readmission.   This employer offers flexible scheduling and encourages a work/life balance.
